Request An Application By Mail Or Fax

1. Click HERE to request an application by mail or fax.
2. Complete the form and submit.
3. An application will be mailed or faxed to you shortly.

1. Click on the "Property Locations" button above.
2. Choose your preferred search method - City or County
3. Choose the city or county you are interested in from the drop down menus.
4. Once you have found the property's page that you want an application for, scroll down to the    bottom of the property's page and click the words "Print Application".
5. Follow the application instruction page that prints with the application and mail, fax or scan/email the application to us.

Important Notes:

• You must use the application that is available on the property's page that you want to apply for.  Different properties require different applications.
• All adults 18 and older must complete and sign the application.
• You will need to have Adobe Acrobat Reader® to print or view the application.  You can download it for FREE from the property's page.
• Incomplete applications will be returned!
